Under the Table

When it became clear he wasn't going to inherit, one of Pegasus's adopted orphans/potential heirs swiped some especially rare cards and used them as bribes to convince some less reputable duelists (eliminators, mercenaries, store breakers, etc.) to help him set up a secondary dueling circuit, based around the option of gambling on the results. With that in the works he blackmailed a KaibaCorp tech worker to leak the in-progress Duel Disk technology so a larger, better Battle Box could be made.
These new arenas were termed 'cages' for their closed-in design, and how when the gambling & better holograms didn't prove enough, a darker practice was put into place. Players were locked in and hooked up to the system and electrocuted in tandem with LP loss, and some would attempt to escape or surrender during the game. More than a few died in the earliest days, when the invention was unrefined.
Being "pirated" in the first place, the cages vary in design and torture-method based on where they are found. To keep them under wraps, they're installed solely in already disreputable areas, so anyone who sees them or plays will be wary of revealing their own participation, or drawing the wrath of the game hosts. The players, gamblers, and game hosts are usually referred to as Backroom Gamers, Bettors, and Dealers, and being invited to join in to play or bet is referred to as "stepping out back".

Black Clown is a decades old casino, once widely known. But it went near-under when the owner, Mr. Otogi, played a dark game with one Mutou Sugoroku to get hold of a golden puzzle box. He lost, had to hide his face under a mask, and Black Clown went nearly broke. He managed to salvage it, but couldn't let go of his need for vengeance, and thus raised his only son to be a master gamer.
The plan was for Bones to beat Sugoroku - and eventually his grandson - at a game of his own design. But being casino owners, they were approached to become Backroom Dealers in the cage circuit. Seeing the horrific aspects of the game as a wonderful means of enacting his revenge, Mr. Otogi insisted on changing plans, agreeing to set up a cage in his casino for both the wild profits, and as a means of trapping Mutou Yuugi.
Bones was originally wary of this, given Yuugi just beat Pegasus at his own game, and thinking they had a better chance by finishing his own game design and using that. But he quickly became addicted to the high stakes, focusing most of his attention on mastering the game. He's become a favorite of the circuit, as people love watching him play, and seeing the custom cages he designs. He uses his own recklessness and confidence to attract other duelists into the cage, most as his opponents but from time to time as his "stand-ins" - namely when a recent loss left him ordered to avoid the shocks for a while.
But his games are all still focused around his lifelong lessons of revenge, and his father grows impatient to get to the real prize, before Bones' games of Duel Monsters Russian Roulette get him killed.
